Registration



REGISTRATION AT JEFFERSON MIDDLE SCHOOL

New student registration will take place at Jefferson August 12th - 13th, 8:30a.m. - 11:30a.m. and 1:00p.m. - 3:00p.m.  No appointment is necessary. Please be sure to have all necessary paperwork. Please be aware that a biological parent/legal guardian must be present to register the child.

What items should I bring when I register my child(ren)?

  • Proof of Residence
    1 of the 2 items below:

    • mortgage agreement (new within the last 30 days) OR
    • electric or water bill (within the last 30 days)

 

  • Driver's License, Texas ID or Military ID

 

  • Withdrawal Form - if enrolling during the school year

     

  • Report Card (most recent)

     

  • Birth Certificate - official copy

     

  • Social Security Card

     

  • Immunization Records

     

  • Standardized Test Scores (STAAR) - or any other Testing Data available

     

  • ARD/IEP copies (most recent) - only if child receives spec. ed. services

     

  • Divorce and/or Legal Guardianship paperwork (if applicable)
