REGISTRATION AT JEFFERSON MIDDLE SCHOOL

Registration for families new to the Jefferson M.S. attendance area will take place daily. No appointment is necessary. Please be sure to have all necessary paperwork. Please be aware that a biological parent/legal guardian must be present to register the child.

What items should I bring when I register my child(ren)?

  • Proof of Residence
    1 of the 2 items below - NOTHING ELSE CAN BE ACCEPTED!

    • mortgage agreement (new within the last 30 days) OR
    • electric or water bill (within the last 30 days)
  • Driver's License, Texas ID or Military ID
  • Withdrawal Form - if enrolling during the school year

  • Report Card (most recent)

  • Birth Certificate - official copy

  • Social Security Card

  • Immunization Records

  • Standardized Test Scores (STAAR) - or any other Testing Data available

  • ARD/IEP copies (most recent) - only if child receives spec. ed. services

  • Divorce and/or Legal Guardianship situations - Details (see #10)
